  • Academy of Sport and Wellbeing Members' Safety
  • Making sure that the Academy of Sport and Wellbeing is a safe environment for all members.  This includes making sure that individual members can safely use the Academy of Sport and Wellbeing, by checking for members' health.
  • Exercise prescription based on Par Q and (medical clearance from GP, if required).
  • Complying with health and safety obligations.

Purpose 3:

Marketing

  • Academy of Sport and Wellbeing marketing communications.

Purpose 4:

  • Track and Trace
  • Facilitation of the Government “Test & Protect” system.

Our legal reasons for using the data are:

Our legal reason to use the data is:  Use is necessary for the performance of a contract with you to take steps, at your request, before entering into such a contract.  That contract being the membership agreement between the Academy of Sport and Wellbeing and Academy of Sport and Wellbeing members. Facilitation of Government Test & Protect.

The data we use includes special category (sensitive) data; your health data.

Our legal reason for using this sensitive data, as provided, is:

  • We have your explicit consent to use this data.
  • We use your health data to make sure you can use the Academy of Sport and Wellbeing without it presenting an unreasonable risk to your health or the health of others.
  • If you were to withhold the personal information we require for the Membership Agreement, the consequences would be:
  • The Academy of Sport and Wellbeing would not be able to process your membership agreement.  This means that the Academy of Sport and Wellbeing would be unable to provide you with its services or allow you access to its facilities.
  • Your data will, or may, be shared with the following recipients or categories of recipient:

Academy of Sport and Wellbeing is part of Perth College UHI and will not share your data with anyone.

Your data will be retained for the following period:

  • 3 years – Purpose 3
  • 7 years – Purpose 2 (fin)

THIS FORM MUST BE COMPLETED BY THE PARENT OR LEGAL GUARDIAN OF THE JUNIOR GYM MEMBER (AGED 7-16)

All new customers to the Academy of Sport and Wellbeing must complete this Physical Activity Readiness Questionnaire (PAR-Q) prior to undertaking any physical activity.

Gym User Conditions

Children between the age of 7 and 16 can only use the cardiovascular (CV) equipment in the fitness rooms if supervised by an adult or a ASW member of staff as part of a fitness session. The adult responsible for the child must complete the Induction with the child. Only the adult who has completed the induction with the child is authorised to supervise the child in the gym. Any other adult who wishes to supervise the child must complete an induction with the child.

Use of any equipment other than the cardiovascular (CV) by any child between the age of 7 and 16 is strictly forbidden.  Any strength training advice or exercises can only be issued by an ASW instructor. This is an appointment which must be arranged by YOU the responsible adult. Exercise sessions demonstrating any strength exercises must be delivered in full by the ASW instructor to the child and adult responsible. No other exercises other than that on the programme should be carried out

Supervision Declaration

By signing below I am stating that I am:

  • I hereby acknowledge that I am fully responsible for the junior named on this form
  • I am fully responsible for ensuring the named junior behaves in an appropriate manner and uses equipment safely as demonstrated in the induction or as part of a Fitness class.
  • If I have any concerns regarding this, I will ask for help/advice when required. 
  • I can confirm my child is between the age of 7 and 16
  • When supervising this junior myself I will always have them within eye contact and be in the same room as them.
